If you can’t locate your EIN, you can contact the IRS at 1-800-829-0115 to obtain it. Be prepared to verify your identity as a corporate officer.
Item 3: Proof of Your State Tax ID Number and Current Year RatePlease provide documentation showing your State Tax ID Number and the current employer tax rate. This information can be found on your Tennessee state unemployment filings or any state-issued correspondence (such as the Tennessee Quarterly UI Tax Return).
Item 4: Employee InformationFor each active employee, please provide the following:
W-4 details (name, address, SSN, marital status, dependents, hire date, birth date, pay rate, deductions).
Signed Direct Deposit Authorization form and a copy of a voided check for each employee choosing direct deposit.
